Revamped lead graphics

-revamped lead graphics
-added remnant lead chest graphics
-moved old files to a folder
-moved stacked icons to different folder
-changed mod icon
-added mipmaps to all icons
-optimized size with pnggauntlet (lossless)
-changed mod version to 0.3
-Updated changelog
